Emergencies
For life-threatening emergencies, please call 911 or go to the nearest emergency department. Messages sent through the patient portal are not monitored in real time and responses may take up to 24–48 hours. For urgent concerns, please contact your local 24/7 crisis line at 360-586-2800 or the National Suicide Prevention Lifeline at 1-800-273-TALK (8255).
Intake Form Policy
All new patients are required to complete the intake forms at least 24 hours prior to their initial evaluation. Failure to complete required paperwork in advance may result in rescheduling of the appointment.
Cancellation Policy
Appointments must be canceled or rescheduled at least 24 business hours in advance for weekday appointments (Monday through Friday, excluding holidays). Late cancellations or missed appointments may be subject to a cancellation fee in accordance with practice policy.
